ASSUMPTION OF RISK

I understand that participation in combat sports, martial arts, boxing, Muay Thai, MMA, fitness training, strength and conditioning, sparring, drills, and any other activities conducted by Vanta Combat involves inherent risks, including but not limited to:

• Bruising, cuts, and abrasions

• Sprains, strains, and fractures

• Head injuries and concussion

• Physical contact with other participants

• Equipment-related injuries

• Serious injury, permanent disability, or death

I voluntarily choose to participate in these activities and fully accept all risks associated with my participation.

MEDICAL FITNESS

I declare that I am physically fit and capable of participating in Vanta Combat activities.

I acknowledge that it is my responsibility to consult a medical professional if I have any concerns regarding my physical condition, health, injuries, illnesses, or medications.

I agree to immediately inform Vanta Combat coaches and staff of any injury, illness, or condition that may affect my ability to safely participate.

RELEASE OF LIABILITY

To the fullest extent permitted by law, I release and discharge Vanta Combat, its owners, directors, coaches, trainers, staff, volunteers, contractors, and affiliates from any liability, claims, demands, actions, damages, costs, or expenses arising from or connected to my participation in any Vanta Combat activity.

This release includes claims arising from negligence, except where liability cannot legally be excluded under Australian law.

PROPERTY DAMAGE

I understand that Vanta Combat is not responsible for any loss, theft, or damage to personal belongings brought onto training premises or training locations.

I accept full responsibility for my own personal property.

PHOTOGRAPHY AND MEDIA CONSENT

I grant Vanta Combat permission to photograph, film, and record me during training sessions, events, and activities.

I understand that these images and recordings may be used for:

• Social media content

• Marketing and advertising

• Website content

• Promotional materials

I waive any right to compensation for such use.

MEMBERSHIP PAYMENT TERMS AND LEGAL COMPLIANCE

I acknowledge that if I enter into a 6-month or 12-month membership agreement with Vanta Combat, I am legally responsible for making all payments due under that agreement for the full membership term.

I agree to maintain valid payment details and ensure sufficient funds are available for all scheduled membership payments.

If any payment is declined, dishonoured, reversed, or otherwise fails, I authorise Vanta Combat to contact me regarding the outstanding amount and to take reasonable steps to recover the debt.

I understand that failure to rectify overdue payments may result in suspension or cancellation of membership privileges and that any outstanding debt may be referred to a debt collection agency or pursued through legal means in accordance with applicable Australian laws.

I agree to comply with all membership terms, payment obligations, and applicable legal requirements relating to my membership agreement with Vanta Combat.

By signing this waiver, I consent to the use of my image and recordings as described above unless I notify Vanta Combat in writing that I do not consent.

RULES AND CONDUCT

I agree to follow all instructions provided by Vanta Combat coaches and staff.

I understand that unsafe behaviour, misconduct, aggression outside training expectations, or failure to follow instructions may result in immediate removal from training sessions or cancellation of membership without refund.

INDEMNITY

I agree to indemnify and hold harmless Vanta Combat, its owners, coaches, staff, and affiliates against any claims, damages, costs, or expenses arising from my actions, conduct, or participation in Vanta Combat activities.

If the participant is under 18 years of age, a parent or legal guardian must complete the following section.

ACKNOWLEDGEMENT

I acknowledge that I am signing this document voluntarily and understand the legal rights I may be giving up by signing it.
